You know I really don't think this is a problem in written Lojban,
because you can always go back and add "goi"s when you need to. And
hopefully in informal spoken lojban, just as in informal spoken
English, ambiguity like that would be par for the course. I rarely
bother to speak more precisely, and when I do, I'm usually thinking
hard enough about what I say that (hopefully) forethought or sumti
counting would be possible. I'd like to think the problem will be
solved when we all get neural implants that can take care of
backcounting since our brains are naturally so bad at it.

I just read a Peter Watts novel1 so I'm all hyped up on neural implants.
